Please download new patient forms and fill out as much information as you can prior to your first office visit. Please arrive 15 minutes early and bring with you your most current insurance card. Also, make sure you have the most current list of medications you are on along with the dose of your medications. If your Primary Care Provider referred you for a consultation, please bring with you all records which pertained to your consultation. For example, if you are consulting for an ovarian cyst, make sure that you have imaging reports that diagnosed you of your ovarian cyst such as a CT scan or a pelvic ultrasound. Without those records, we may be limited with the information we have and the consultation is less productive. We may be forced to repeat unnecessary tests that you are unsure about and whether those tests have been performed by your Primary Care Provider. If you are seeking a second opinion, please bring your medical records with you regarding your medical findings and your doctor's proposed treatment.
